Immunization Requirements

Minnesota law requires all students in Early Childhood through Grade 12 to have up-to-date immunization records or a notarized exemption form on file. Your child cannot start Early Learning Preschool or ECFE classes without this documentation. If you have questions, please call us at 952-928-6774.

Need to Submit an Exemption Form? We’re here to help! If you choose to exempt your child from immunizations, you must complete an exemption form in the presence of a notary public. An on-site notary is available at Door 1 of the Central Community Center. General availability is Monday–Friday,  8 a.m. to 4 p.m. A notary is also available in the Central Early Learning Center main office most afternoons from 12 p.m. to 4 p.m., Monday-Friday.
